Resumen
Isolated cortical vein thrombosis is a rare entity and remains a diagnostic challenge mainly due to variations in number, size and location of these veins. We present a female 60 yo patient that presented with headache, followed by ictal episode, with admission non enhanced brain CT showing subarachnoid hemorrhage in the left sylvian fissure. The diagnosis of cortical vein thrombosis was made on brain MRI, mainly magnetic susceptibility secuence, because of its great sensitivity to paramagnetic hemoglobin products at different stages of clot formation,.
