Resumen

Avascular necrosis of the bones has been thoroughly studied and reported. Vascular patterns have identified the scaphoid and lunate bones as at particular risk (Gelberman and Menon, 1980). On the other hand, the vascular anatomy of the pisiform has been considered to be rich, with plentiful arterial and capillary anasto- moses.
We report a case of spontaneous symptomatic necrosis of the pisiform, associated with a ganglion cyst originating from the pisotriquetral joint and pisotrique- tral arthritis in an otherwise healthy, sedentary middle aged woman.
