Background, climate has been associated with health and disease, extreme climate variation may be increase incidence of diseases, use of medical care services and health spending. ENSO phenomenon causing increased rainfall and temperature changes. The Colombian´s Health Care System, pay for each insured person a fixed amount per year to Health Insurance Compay, this amount, is called, Unidad de Pago Por Capitacion UPC. UPC is not adjusted by extreme climate variation. The influence of climate changes in the incidence of diseases and use of health care services could produce a financial imbalance in the system. The objective is to define the rainfall and temperature level that produce significant deviation in the incidence of disease, use of health care services and health spending. Methods, a cohort study will be conducted and the meteorological, mobidity, use of health services and cost of health services data during 2007-2011 in a population affiliate to a insurance health company in the Valle del Cauca State, will be studied to analyze the statistical relationship between meteorological data time series and disease incidence, use of health services and cost of health services data series. Temporal correlation between incidence of diseases, use of health care services, cost of health services and meteorological factors will be analyzed using multiple lineal correlation.
