Abstract

Introduction: the prevalence of heart failure with preserved and mildly reduced ejection fraction is high and is associated with increased hospitalizations, cardiovascular mortality, and all-cause mortality. Evidence regarding the optimal treatment for these patients can be controversial; thus, a high level of clinical suspicion and education is required to establish early and accurate treatment. Objective: to conduct an expert consensus on the treatment of patients with heart failure with preserved and mildly reduced ejection fraction, in charge of the Chapter of Heart Failure, Heart Transplantation and Pulmonary Hypertension of the Colombian Society of Cardiology and Cardiovascular Surgery (SCC). Materials and method: 27 cardiologists from the Chapter of Heart Failure, Cardiac Transplantation, and Pulmonary Hypertension of the SCC were selected. Twelve questions were defined to be addressed during the consensus. A literature review related to each of the questions was conducted. The consensus was developed in nine phases, and the Nominal Group Technique was used. Results: Twelve recommendations were proposed regarding the management of patients with heart failure with preserved and mildly reduced ejection fraction, along with a precise diagnostic algorithm for the evaluation of patients suspected of having this condition. Conclusions: This consensus addresses the need for clear guidelines to improve the diagnosis and treatment of patients with heart failure with preserved and mildly reduced ejection fraction.
